In a couple weeks you will begin working on your final project for winter term. The purpose of this project is to empower students to advocate for their health and lifelong wellness. One of the best tools we have for preventing illness and disease over our lifetimes is the knowledge of our family history. As part of the project, you are asked to gather information about medical conditions experienced by members of your immediate and extended families. Students who may have little or no access to information about blood relatives can complete this research on their adopted or guardian family. Students who were donor conceived are also welcomed to complete this research from the parent(s) in your life. Besides genetics, our shared environment, habits and cultural traditions also play an important role in influencing overall health. The information gathered in this project is for the student, not for me their teacher. You will use this information to inform your final project, but I won't receive a copy of any of the answers provided by your family. This will be your personal data to analyze.

This google form will will help you learn about your family’s health history and get ideas for your final project.
