Request for Immunization Status letters to our families with students currently missing immunizations have been mailed. Exclusion day is February 21, 2018 this year.
Some families may ask you where and when they can get their immunizations, so I have attached a couple flyers, as well as the list below of some of the options.
1. The Primary Care Provider for the student
2. Either of the Clackamas County Health Department Clinics: Call 503-655-8741 for an appointment.

3. Outreach Clinics:
· Jan 10 @ Wilsonville…clinic hours, 4-6:30 p.m. See attached flyer for location and other details.
· Feb 7 @ Oregon City…clinic hours, 5-7 p.m. Providence WFH Community Center, 519 15th St.
· Feb 14 @ Oregon City…clinic hours, 5-7 p.m. Providence WFH Community Center, 519 15th St.
Lastly, if parents do not want to immunize their children, they may go to the Vaccine Exemptions page of the Oregon Health Authority website to find links to the appropriate exemption information, including the education modules.
