HOLIDAY FOOD BOXES!
Forms are on Crystals desk and are also attached in English, Russian, Spanish & Vietnamese.
Please turn them in to Crystal by December 4th
before 3:00pm
All forms/information will remain confidential.
**First come first serve, the sooner you get your form in the better**
Here are some important details:
-Applications will be accepted starting on Friday, November 1st. I will turn them in to Family Support no later than December 4th.
-Deliveries will go out on December 21st. They start pretty early in the morning (around 8am). It's important that families are home, or they may not receive their items
-Toys are provided for families who have kids under the age of 14. There is a column on the form that says "toy request." Parents completing the form can request a type of toy based on their kid's interest
-For people who can't have a box delivered for various reasons (don't have an address, are sharing housing with others, or their address is outside of our school district boundary, etc) the Wichita address can be used instead. Our address is 6031 SE King Rd. Milwaukie, OR 97222. We will work with these families to arrange pick up or a special delivery
-Only one address can be used per form, so if people are sharing housing they would need to apply together or use Wichita as their address
